Fill Out the Eligibility Checklist

Once you’ve determined that you’re qualified, and read through the information on our website, fill out our short Eligibility Checklist. Once you complete the checklist, the link to our online application will be revealed.

Application Guidelines

Check the boxes below if you organization does fit our guidelines. If you do not, be aware that your organization is not qualified to submit an application.

Yes, our organization is a public charity, organized under IRC Sec. 501 (c)(3), and qualified under IRC sec. 170 (b)(1)(a), OR an organization to which contributions are tax deductible, which includes veterans organizations

Yes, we provide services within Minnesota and/or to Minnesota residents being treated out-of-state by the following medical facilities: Fargo, ND; Sioux Falls, SD; or Tomah, WI VA Medical Centers.
